LIVING WILLS
| We can provide
you with a straightforward form of Living Will, also known as an
Advance Directive, which states your wishes for medical care if
you are unable to tell your doctor personally because you are incapable
as a result of your medical condition. You can take it to your doctor
and your family, to discuss together before you complete it. The
form should be kept with your medical records. |
HOME VISITS
| We are always
happy to come to see you at home if this is more convenient for
you for time, mobility or other reasons. We may have to make an
extra charge to cover the time spent travelling but it is important
that your needs are met. |
